How can I request a copy of my medical records?

Clients are required to come in person to request their medical records. No medical records will be provided in digital or electronic format. The individual or next of kin must visit the Hospital Administration Office to obtain the medical record. Proper identification will be required for verification and authentication. Processing takes approximately 3 to 7 business days.

Is there a fee for obtaining my medical records?

There is no fee for obtaining your medical records. Once the verification process is complete, the records will be issued accordingly at no cost.

How long does the hospital keep my medical records?

Our hospital retains medical records indefinitely in accordance with health regulations. You may request access to your records at any time by following the approved procedure.

Can I request my medical records to be sent to another healthcare provider?

No, due to privacy laws we can’t forward your medical records to another registered healthcare provider. However, both you and the receiving healthcare provider must formally request the records through the Hospital Administration Office. The same in-person verification procedure applies.
